Purpose and Intended Use:

Equine form contracts designed for riding instruction programs of all sizes, from one person providing a few lessons to large equestrian centers.  Not designed for horses in training except in lessons with the client – see Equine Legal Solutions’ Professional Training Agreement Package. 

Included Riding Lesson Forms:

Six separate forms:

• Riding lesson application form and associated instructions 
• Separate riding lesson agreement forms for youth and adults (includes liability releases and authorization for emergency medical treatment)

Document Format:

• Each agreement is a fillable, savable PDF.
• Each horseback riding lesson form comes with detailed instructions on how to fill out and understand each portion.

The Basics:

• Clauses in every Equine Legal Solutions contract. 
• Client’s riding experience and goals.
• Contact information for client and emergency contact.
• Parent’s consent and role in lesson program (in youth agreement).

Lessons:

• Can check off options and prices for private, semi-private and group riding lessons.
• Riding lesson cancellation policy
• Section for competitions, including payment and reimbursement of fees, with different options.
• Suggested additional services that riding instructor may want to offer (can delete if you don't plan to offer them).

Payment Terms:

• When payment is due.
• What makes a payment late.
• Penalties and interest for late payments.
• Fees for returned checks and other dishonored payment instruments.
• Payment/reimbursement of competition-related expenses.  
• Lesson cancellation fees.
• Provisions for commissions on sales and leases of horses for client.

Authorization for Medical Treatment:

• Client (in youth agreement, parents) consent to medical treatment and provide emergency contact information

Liability Release:

• Client (in youth agreement, parents) assumes all risks of loss or injury to self, personal property and Client’s horse. Includes provisions regarding helmets and other safety attire. 
• Client (in youth agreement, parents) agrees to indemnify instructor if instructor is sued.
• Instructor's liability limited to amounts that instructor has received under the contract.
