Purpose and Intended Use:

Equine form contracts designed for riding instruction programs of all sizes, from one person providing a few lessons to large equestrian centers.  Not designed for horses in training except in lessons with the client – see Equine Legal Solutions’ Professional Training Agreement Package.

Riding Lesson Client Intake Form Contents:

  • Client’s name and contact information. 

  • Whether client is under 18, and if so, name and contact information for client’s parent or guardian.

  • Whether client has any physical limitations or other special considerations.

  • Client’s riding, horse ownership and competition experience.

  • Client’s goals.

  • Client’s day/time preferences for lessons.

Adult Riding Instruction Agreement Contents:

Basic Provisions

  • Clauses in every Equine Legal Solutions contract.

  • Client represents they are at least 18 years old.

  • Client represents they don’t have any physical or mental conditions that might prevent them from safely riding and handling horses.

  • Client represents they aren’t under the influence of drugs or alcohol, nor will they be at any time during the lessons.

Lesson Program Options

  • Can check off options and prices for different types of lessons, including private, semi-private and group riding lessons.

  • Includes an “other” option to describe unique lesson program offerings.

Liability Releases and Disclaimers

  • Client assumes all risks that they will be injured or killed, and agrees not to sue instructor or facility owners. Includes specific provision for trail riding and other forms of riding outside an arena.

  • Client assumes all risks that personal property, such as tack and equipment, will be lost or damaged. Agreement specifically states client is responsible for insuring their own personal property.

  • If client uses their own horse for lessons, client agrees to accept all risks that horse will be injured or killed. Agreement specifically states client is responsible for insuring their own horse.

  • Instructor recommends ASTM/SEI certified helmet and other safety attire, and if client doesn’t wear it, client agrees to assume increased risk that client will be injured or killed. (Why agreement doesn’t require helmets and other safety attire.)

  • If instructor or facility is sued by a third party in connection with the client, such as the client’s health insurance company, the client agrees to indemnify the instructor/facility and pay for their legal defense.

  • Client agrees that instructor’s and facility’s total liability won’t exceed what client paid to instructor for lessons.

Authorization for Medical Treatment

  • Client agrees that instructor can seek medical treatment in an emergency, and client agrees to pay for medical treatment, regardless of whether client’s insurance covers the cost.

Lesson Cancellation Policy

  • Riding lesson cancellation policy, with customizable time periods for cancellation notices.

Payment Terms

  • Can specify when payment for lessons and other services are due.

  • Can specify when payment will be considered late.

  • Can specify late fees and interest on past due balances.

  • Can specify fees for returned checks and other dishonored payment instruments.

Shows and Competitions

  • Covers going to competitions, including customizable sections for payment of show fees.

Suggested Fee Schedule

  • Can use included suggested fee schedule or substitute your own.

  • Includes customizable options for lesson packages and prices.

Youth Riding Instruction Agreement Contents:

Basic Provisions

  • Agreement is signed by parent or guardian, and they represent they are an adult with authority to sign the agreement on behalf of the child.

  • Parent or guardian represents that the child doesn’t have any physical or mental conditions that might prevent them from safely riding and handling horses.

  • Includes an “other” option to describe unique lesson program offerings.
